All forms are available for download using Adobe Acrobat software. Adobe Reader 8 or higher will also allow you to complete the forms online and save a completed copy for your files. Your center director can answer any questions you may have as well as provide any additional forms that may need to be completed.

1. Child Care Application for Enrollment (REQUIRED)

To be completed and signed by child’s parent/guardian. Includes child and family information as well medical information and persons allowed to pick-up your child.

2. Child Information Form (Required)

Specific family information and identifying information about the child.

4. Hillsborough County Child’s Enrollment/Information Form (Required)

To be completed and signed by child’s parent/guardian. Includes pick-up authorization, emergency contact information and authorization for emergency medical treatment. This document may be notarized.

5. Hillsborough County Know Your Child Care Facility (Required)

This brochure provides families with helpful information regarding child care facilities including the quality indicators of a child care facility, the parent’s/guardian’s role in quality care, and some of the minimum standards used to license child care facilities.

6. Hillsborough County Medication Authorization Form (Required)
Complete for any prescription or non-prescription medication to be administered to your child while at the center.
7. Know Your Child Care Facility (Required)
Brochure to read highlighting the responsibility of the parent and the center to families to provide a quality child care center including general and training requirements, physical environment and a food and nutrition requirements.
8. Medication Authorization Form (Required)

To be completed by parent/guardian and child’s medical provider (when necessary), authorizing the administration of medication to child while at the center.

9. Sample Certificate of Immunizations (Required)

Record of child’s immunizations to be completed and signed by child’s health care provider (will be provided by the child’s health care provider).

10. Sample School Entry Health Exam Form (Required)

Sample Form Only-To be provided by child’s medical professional. Part I to be completed and signed by child’s parent/guardian; Part II to be completed and signed by child’s health care provider.

11. Personal Care Plan/Developmental History (Required)

To be completed by family at time of enrollment or when child transitions to the next age group. Provides detailed information to help provide the best care possible to your child.

Tips and reminder to always check the back seat of your car before locking the door and walking away. Complete and return the acknowledgement to center management.
Flu Guide for Parents

Brochure to provide families with information detailing the causes, symptoms, and transmission of the influenza virus (the flu). Review, sign and date the signature page and return to the center.

Permission for Food-Related Activities & Special Occasion Food Consumption

Florida requires written permission from parents/guardians regarding a child’s participation in food related activities such as classroom cooking projects, gardening, and celebrations. Please review, complete and sign.
